Case Manager

Department: 082 - Road to Home
Location: Leesburg, FL

JOB SUMMARY: The case manager functions as an advocate and support person to individuals admitted to the Road to Home program.

ESSENTIAL FUNCTIONS: The list of essential functions, as outlined herein, is intended to be representative of the tasks performed within this classification. Advocates in the clientís best interest. Provides holistic/comprehensive case management services to all consumers including intake, benefit assessment, goal setting, long term case plan development, weekly case plan development, progress monitoring, individual money management and advocacy and referrals.  

With the individualís permission, involve significant others in individualís treatment. Link individuals to needed services. Monitor and evaluate service provision. Coordinate Medication Clinic appointments of all individuals on the assigned caseload. Convene/attend staffing, as necessary, for assessing and coordinating services for individual. Coordinate the discharge of individuals into the community. Work with other case management staff (if applicable in other counties) to link, advocate, refer and place individuals in the most appropriate post program discharge situation. Ensures that all documents are valid and if applicable work with the individual to secure entitlements and other income sources. Participates in regular staff meetings, staff training programs, supervisory sessions, and accepts the responsibility for aiding the development of positive team relationships as requested. Adheres to the agency policy and procedures and the professional code of ethics. Connects individuals with needed and available community resources. Inputs accurate and complete data for all contacts and services into the electronic health record.

Assesses needs and develops a plan to meet those needs. Convenes/attends staffing as necessary for assessing and coordinating services for clients. Completes required Center, DCF, legal, statistical and fiscal documentation in an accurate and timely manner. Complete Center and DCF Training. Must have a valid driverís license and a clean driving record.

RESPONSIBILITIES: The population is made up of individuals who suffer from severe and persistent mental illness and/or have multiple problems and needs that have been recently discharged from the State Hospital. The case manager works closely with the individual, the individualís family, Center staff, and other community organizations, to meet the identified needs and to allow the individual to live as independently as possible within the community. Develops relationships with community providers and resources to develop a strong resource base for program participants.

KNOWLEDGE AND JOB SKILLS: Must have a working knowledge of billing procedures, requirements for reimbursement and pertinent state regulations. Ability to make clinical and administrative decisions. Ability to develop and maintain community network of resources. Good clinical skills with the ability to make clinical decisions about the individualís stability are basic. Time management skills are fundamental. Good verbal and written communication skills are vital to this position. Good handwriting and documentation skills are essential. Professional telephone skills are essential. Computer knowledge is vital to the satisfactory performance of this position.

JUDGMENT: Sound clinical judgment as demonstrated by the ability to make on the spot decisions based on the individualís current statues, past functioning level and potential strengths and weaknesses is essential to job performance. The ability to determine appropriate resources to meet the needs of the individual.

JOB REQUIREMENTS:

Education: Bachelorís Degree with major course work in the human services field.

Experience: A minimum of one (1) year experience in Behavioral Healthcare is required. Equivalent training and experience on a year-for-year basis in a related field will be considered.  

Personal Qualities: Patience and high tolerance for frustration is essential. Initiative, dependability and accuracy are important. The ability to analyze situations and mange them in a proactive manner is helpful. Must be able to work with a minimum of direct supervision.

Special Factors: Must be able to meet deadlines. Must be able to document accurately and remain current with all documentation. It is important for a person in this position to enjoy working with difficult clients and act as a role model whenever possible. Able and willing, to work flexible hours. Must have reliable transportation and be willing to drive as a part of their basic job duties.

Staff Development: Completes all administrative and requirements as set forth in the Individual Training Plan. Required by DCF to have twenty (20) hours of staff development training per year.
