Adult Case Manager

Department: 029 - Adult Case Management
Location: Leesburg, FL

ESSENTIAL FUNCTION: Advocates in the clientís best interest. Conduct a holistic assessment of individualís living situation at a minimum of every six (6) months, and more often, when there are significant changes in the individualís situation. Schedule at least one (1) monthly face-to-face contact with each individual, preferably in his or her natural living environment.

With the individualís permission, involve significant others in individualís treatment. Link individuals to needed services. Monitor and evaluate service provision. Coordinate Medication Clinic appointments of all individuals on the assigned caseload. Convene/attend staffing, as necessary, for assessing and coordinating services for individual. Coordinate the discharge of case management individuals in the Centerís Psychiatric Hospital.

Via the State Hospital Liaison Case Managers provide information for discharge, case coordination and complete required Center and DCF forms.

Assesses needs and develops a plan to meet those needs. Convenes/attends staffing as necessary for assessing and coordinating services for clients. Coordinates the discharge of case managed clients from the Centerís Psychiatric Hospital or other inpatient or residential settings. Completes required Center, DCF, legal, statistical and fiscal documentation in an accurate and timely manner. Complete Center and DCF Training. Must have a valid driverís license and a clean driving record. 

RESPONSIBILITIES: The case manager functions as an advocate and support person to those Center individuals meeting the definition of a ďpriority clientĒ as defined by Chapter 65E-15. The population is made up of individuals who suffer from severe and persistent mental illness and/or have multiple problems and needs. The case manager works closely with the individual, the individualís family, Center staff, and other community organization, to meet those needs and to allow the individual to live as independently as possible within the community.

KNOWLEDGE AND JOB SKILLS: Must have a working knowledge of billing procedures, requirements for reimbursement and pertinent state regulations. Ability to make clinical and administrative decisions. Ability to develop and maintain community network of resources. Good clinical skills with the ability to make clinical decisions about the individualís stability are basic. Time management skills are fundamental. Good verbal and written communication skills are vital to this position. Good handwriting and documentation skills are essential. Professional telephone skills are essential. Computer knowledge is vital to the satisfactory performance of this position.

JUDGMENT: Sound clinical judgment as demonstrated by the ability to make on the spot decisions based on the individualís current statues, past functioning level and potential strengths and weaknesses is essential to job performance. The ability to determine appropriate resources to meet the needs of the individual.


Education: Bachelorís Degree with major course work in the human services field. Certification as a targeted case manager required within the first six months of hire. 

Experience: A minimum of one (1) year experience in Behavioral Healthcare is required. Equivalent training and experience on a year-for-year basis in a related field will be considered.  

Personal Qualities: Patience and high tolerance for frustration is essential. Initiative, dependability and accuracy are important. The ability to analyze situations and mange them in a proactive manner is helpful. Must be able to work with a minimum of direct supervision.

Supervisory Responsibilities: None

Special Factors: Must be able to meet deadlines. Must be able to document accurately and remain current with all documentation. It is important for a person in this position to enjoy working with difficult clients and act as a role model whenever possible. Attends clinical supervision with clinical supervisor on a monthly basis. Able and willing, to work flexible hours. Must have reliable transportation and be willing to drive as a part of their basic job duties,

Staff Development: Completes all administrative and competency requirements as set forth in the Individual Training Plan. Required by DCF to have twenty (20) hours of staff development training per year.
